We are excited that you have chosen us to partner with you in your oral health journey.
Before your first visit, we ask that you fill out the following forms and submit them online for us to process:
- Intake form - Policy
- Treatment Consent form
- Medical History form
- Dental History form
- Child Health History form (If your child will also be a patient)
Your First Appointment
During your first visit, we perform a comprehensive diagnostic exam and consultation. It’s important for us to assess the current state of your oral health, and identify any existing issues or reasons for concern.
We include:
- A check for tooth decay.
- Screen for periodontal (gum) disease.
- Screen for oral cancer and other health-related oral issues.
- An assessment for tooth grinding (bruxism) or toothbrush abrasion.
- A review of old crowns, bridges, and fillings for any deterioration.
- A check for dental occlusion (bite - how your top and bottom teeth come together)
- Complimentary iTero intraoral scanning.
Once this examination is complete, we meet with you to discuss our findings and provide any recommendations we have. We work with you to understand what each recommendation includes, how much it will cost, and what the impact will be on your everyday life (number of appointments, healing/recovery time) as necessary. We want you to make the best, most informed decisions about improving your oral health.
